J’autorise mon enfant à participer à cet évènement, organisé par L'Office de l'Éducation à la Foi et Mission Jeunesse de la Corporation archiépiscopale catholique romaine de Montréal (CACRM). Je confirme que la participation de mon (mes) enfant(s) à cet événement est volontaire. J’accepte que mon enfant respecte entièrement les consignes, et le code de conduite qui seront communiqués au début de la session, ainsi que toutes les lois municipales et provinciales applicables. Je confirme que toutes les informations fournies dans ce formulaire d’inscription soient vraies.

1. Je suis responsable d'assurer le transport de mon enfant avant et après l'évènement avec un adulte responsable.

2. Je suis responsable d'assurer la présence de mon enfant à l'évènement avec un adulte responsable.  Aucun jeune ne peut se présenter sans être accompagné d'un adulte.  Je suis aussi responsable d'informer toutes personnes (adultes ou mineurs) qui accompagnent mon enfant des conditions de participation.

3. Advenant qu’un participant ait des allergies, des restrictions alimentaires ou des besoins particuliers, il est responsable de pourvoir aux ressources dont il aura besoin (médicaments, alimentation, fauteuil roulant, accompagnateur avec les capacités nécessaires pour l'accompagner dans le contexte de cette activité, etc.). 

4. Je reconnais que la CACRM peut faire prendre des photos au cours de cette activité et je reconnais que c'est ma responsabilité sur place d'informer les photographes si je ne désire pas que mon enfant soit photographié. Je consens à ce que des images puissent être ainsi utilisées par la CACRM dans des reportages ou à des fins promotionnelles. J’autorise la CACRM - L'Office de l'Éducation à la Foi | Faith Education Office à utiliser librement le nom, la photo, la voix, ou similarités, de mon enfant dans toute diffusion, émission, promotion publicitaire ou autre compte de cet événement, ou marketing ou promotion pour événements similaires futurs, et je renonce expressément à tous les droits au respect de la vie privée qu’il pourrait avoir à cet égard. J'accepte que le CACRM ou tout agent dûment autorisé par le CACRM ne soit pas obligé d'utiliser les photos.  

J’assume tous les risques inhérents à la participation de mon enfant. Par la présente, je renonce irrévocablement à toutes réclamations contre la Corporation archiépiscopale catholique romaine de Montréal (CACRM) leur personnel, leurs agents, leurs représentants, leurs employés, leurs partenaires et leurs bénévoles et toutes autres personnes morales impliquées dans l’organisme et/ou l’opération de cette activité.

 Seul un *nombre limité* d'adultes (parents, tuteurs légaux ou intervenants jeunesse etc...  reconnus par la communauté peuvent accompagner des mineurs selon les directives de la pastorale responsable). CACRM se réserve le droit de limiter la participation d'adultes pour maximiser la participation des jeunes.

Je consens à recevoir périodiquement des communications reliées à la participation de mon enfant à l’événement. 

Je reconnais avoir lu attentivement cette renonciation et quittance et j’en comprends l’intégralité.

I give authorization for my child, to participate in this event organized by Faith Education Office and Mission Jeunesse of the Corporation archiépiscopale catholique romaine de Montréal (CACRM). I affirm that my child's participation in this event is voluntary and I agree that they will abide by guidelines, code of conduct, as well as all applicable municipal and provincial laws and regulations. I confirm that all the information provided in this registration form is true.

1. I am responsible for ensuring the transportation of my child before and after the event with a responsible adult.

2. I am responsible for ensuring my child's attendance at the event with an adult.  Minors will not be admitted if they are unaccompanied.  I am also responsible for informing all persons (adults or minors) accompanying my child of the conditions of participation. 

3. A participant with allergies, dietary restrictions or special needs is responsible for providing the necessary resources to address those needs (medications, food, wheel chair, a person capable of accompanying them in the context of this activity, etc.) 

4. I acknowledge that the CACRM may take pictures (photos) during the course of my child’s participation in this activity, and that it is my responsibility to inform photographers on site if I do not want my child to be photographed. I consent to having pictures in which they appear used by the CACRM in media coverage or for promotional ends. I give permission to CACRM - Faith Education Office/Mission Jeunesse – Youth Ministry for the free use of my child’s name, photograph, voice, or likeness, in any broadcast, telecast, advertising promotion, or other account of this event, or marketing or promotion for future or similar events, and waive any rights of privacy my child may have in that regard. I agree that CACRM or any agent duly authorised by CACRM is not obligated to use the photos

I am assuming all risks that arise out of my child’s participation. I hereby irrevocably waive any liability on the part of the organizers and the CARCM (Corporation archiépiscopale catholique romaine de Montréal), their agents, representatives, employees, partners, volunteers and all other legal entities that may be involved in the organization and/or operation of this activity.

Only a *limited number* of adults (parents, legal guardians, and Youth Ministers recognized by the community) may accompany minors according to responsible ministry policies. Faith Education Office/Mission Jeunesse reserves the right to limit adult participation to maximize the participation of minors. 

I understand and consent that I will be periodically receiving communications related to my child's participation in this event.

I have carefully read this Waiver and Release and fully understand its contents.
